AHA Action Community Informational Webinar 1: Creating Age-Friendly Health Systems
The American Hospital Association (AHA) is holding a webinar June 9 on creating Age-Friendly Health Systems and how to join the movement.
Throughout the United States, hospitals, clinics, long-term care sites and other types of care sites are becoming Age-Friendly Health Systems as they implement the 4Ms Framework - what matters, medications, mentation and mobility - to provide high-quality care that meets the unique needs of older adults. This informational webinar will talk about the importance and impact of the 4Ms Framework, hear bright spots in the field integrating value-based, age-friendly care and understand how to get engaged in the upcoming Action Community convening this fall. Providence St. Joseph Medical Center will share the hospital's story of becoming an Age-Friendly Health System and bridging clinical care and community health to improve the health and well-being of older adults.
