Age Friendly Action Community Informational Webinar

The American Hospital Association (AHA) is holding a webinar August 10 on creating Age-Friendly Health Systems and how to join the movement.

Throughout the United States, hospitals, clinics, long-term care sites and other types of care sites are becoming Age-Friendly Health Systems as they implement the 4Ms Framework - what matters, medications, mentation and mobility - to provide high-quality care that meets the unique needs of older adults. By focusing on the 4Ms, the aim is to improve patient care, safety and outcomes; and reduce length of stay and readmissions.

The webinar will talk about the importance and impact of the 4Ms Framework, hear bright spots in the field integrating value-based, Age-Friendly care and cover the benefits of joining the Action Community. Speakers include:
