American Hospital Association: Advancing Age-Friendly Care - Action Communities Convening
The American Hospital Association (AHA) is holding the Advancing Age-Friendly Care: Action Communities Convening, May 12-14 in Chicago, to bring together Age-Friendly Health Systems Action Community teams.
This in-person convening will spotlight the innovative work of Action Community teams and highlight the importance of age-friendly care as part of a broader movement to transform how healthcare systems support aging populations. Participants will learn best practices, hear from national leaders in the field, and engage in peer-to-peer knowledge sharing.
In recognition of the progress made and the road ahead, this convening will also offer a unique opportunity for participants to experience the 4Ms (What Matters, Medication, Mentation, and Mobility) in action. On the second day, attendees will take part in a mini site-visit field trip, selecting from one of four different care settings where the 4Ms are being implemented.
