The webinar will introduce the Age-Friendly Health System (AFHS) movement and the 4Ms Framework, and explore how aligning AFHS with broader efforts can strengthen care delivery.
The speaker, Adam Koontz, Sr. Director of Patient Support Services at Novant Health, will discuss how Novant Health used data, team-based strategies, and quality improvement principles to drive adoption, overcome challenges, and build momentum for culture change in the care of older adults.
