BOLD PHCOE-DC Webinar: Guiding and Improved Dementia Experience (GUIDE) Model
The Public Health Center of Excellence on Dementia Caregiving (PHCOE-DC), in collaboration with the Minnesota Northstar Geriatrics Workforce Enhancement Program, is holding a webinar May 27 on "Guiding and Improved Dementia Experience (GUIDE) Model."
The Centers for Medicare & Medicaid Services Guiding an Improved Dementia Experience (GUIDE) model is a collaborative, integrated dementia care program for both Medicare beneficiaries living with dementia and their care partners. Approximately 400 providers are or are planning to deliver GUIDE throughout the US.
The webinar is specifically tailored for people living with dementia, their family, friends, or other care partners, and healthcare professionals wishing to learn more about the resources and support possible in the GUIDE model. The interdisciplinary panel will discuss and answer questions about the GUIDE model.
