The 2025 conference theme was about improving health outcomes through prevention, quality, and safety as CMS transforms healthcare from a system that focuses on sick care to one that fosters prevention, wellness, and chronic disease management.

The John A. Hartford Foundation presented on age-friendly care along with grantees. Aging-related sessions - part of the "Healthy Aging through Prevention and Quality Care" track - included:

  • Driving Change, Delivering Results: Proven Solutions for Better Health (July 2, 3:30pm)
    JAHF's Rani Snyder presented on the Age-Friendly Health Systems (AFHS) movement and age-friendly care across the continuum, including information about the Age-Friendly Hospital Measure and the need for age-friendly nursing home care as promoted by the Moving Forward Nursing Home Quality Coalition.
  • Innovating for Impact: Cutting-edge Solutions in Surgical Care (July 2, 1:30pm)
  • Empowering Aging Tech-driven Tools to Support Function and Wellbeing (July 2, 2:30pm)
