GWEP-Coordinating Center Age-Friendly Health Systems Case Studies Series
Geriatrics Workforce Enhancement Program (GWEP) partners have developed the GWEP Coordinating Center (GWEP-CC) Age-Friendly Health Systems Case Studies series.
The series presents a broad range of cases taking learners through primary care experiences implementing the 4Ms (Mentation, Medications, Mobility, What Matters) of the Age-Friendly Health Systems initiative into practices. The case studies are uploaded on a rolling basis.
