IHI Course: Providing Age-Friendly Care to Older Adults

The Institute for Healthcare Improvement (IHI) has created a free Open School course, "Providing Age-Friendly Care to Older Adults."

Age-Friendly Health Systems (AFHS), an initiative of JAHF and IHI in partnership with the American Hospital Association and the​ Catholic Health Association of the United States, aims to help hospitals, medical practices, retail pharmacy clinics, nursing homes, home-care providers and others deliver age-friendly care.

This course (PFC 203) will teach participants how to provide age-friendly care based on a set of four evidence-based elements, known as the “4Ms”. When implemented together, the 4Ms represent a broad shift by health systems to focus on the needs of older adults.

The course objectives are to:

  • Define age-friendly care
  • Describe the 4Ms framework, which presents four evidence-based elements of high-quality care for older adults
  • Explain how you will assess the current state of the 4Ms in your health system and how to act on those findings to incorporate the 4Ms into routine care
  • List six steps you will take to integrate the 4Ms into standard care for older adults
