Invitation to Join: Age-Friendly Health Systems 2025 Action Community

The Institute for Healthcare Improvement (IHI) is inviting health systems to join the next Age-Friendly Health Systems Action Community that starts in spring 2025.

Age-Friendly Health Systems (AFHS) follows an essential set of evidenced-based practices called the 4Ms: what Matters, Medication, Mentation and Mobility. The free seven-month AFHS Action Community provides a series of monthly webinars and coaching calls to support teams to learn about, test and adopt the 4Ms in their care setting (e.g., a hospital unit, primary care practice, nursing home, and home health) with the goal of being recognized as an Age-Friendly Health System – Participant.

This national Action Community will be the first opportunity for:

  • Hospitals to learn about the Age-Friendly Health Systems movement and become recognized as age-friendly, in service of the new CMS Hospital Age-Friendly attestation measure.
  • Home health care organizations to engage in the Age-Friendly Health Systems movement and become recognized alongside nursing homes, hospitals, ambulatory practices, and convenient care clinics.
