Invitation to Join: Age-Friendly Health Systems Oncology Action Community

The Association of Community Cancer Centers (ACCC) is inviting health systems to join the Age-Friendly Health Systems Oncology Action Community that starts in August 2022.
Throughout the United States, hospitals, clinics, long-term care and other sites of care are becoming Age-Friendly Health Systems as they implement the 4Ms Framework - what Matters, Medication, Mentation and Mobility - to provide high-quality care that meets the unique needs of older adults. ACCC will be holding a free, seven-month learning collaborative that will guide oncology clinical teams in implementing the 4Ms.
The Action Community is a network of teams across different health care organizations that come together to accelerate their own adoption of the 4Ms. Guided by expert faculty and an “all-teach, all-learn” model, teams participate in monthly webinars and attend an in-person or virtual meeting with the goal of learning, testing and sharing strategies to improve care for older adults.
Age-Friendly Health Systems is an initiative of JAHF and the Institute for Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States.
