Invitation to Join: AHA Age-Friendly Health Systems Action Community

AHA Action Comm

The American Hospital Association (AHA) is inviting health systems to join the next Age-Friendly Health Systems Action Community that starts in September 2022.

Throughout the United States, hospitals, clinics, long-term care sites and other types of care sites are becoming Age-Friendly Health Systems as they implement the 4Ms Framework - what matters, medications, mentation and mobility - to provide high-quality care that meets the unique needs of older adults.

The seven-month Action Community is a network of teams across different health care organizations that come together to accelerate their own adoption of the 4Ms. Guided by expert faculty and an “all-teach, all-learn” model, teams participate in monthly webinars and attend an in person or virtual meeting with the goal of learning, testing and sharing strategies to improve care for older adults.

Age-Friendly Health Systems is an initiative of JAHF and IHI in partnership with AHA and the Catholic Health Association of the United States (CHA).
