National Dementia Care Collaborative: Affinity Group Webinar Series on NDCC's Six Evidence-Based Comprehensive Dementia Care Models
The National Dementia Care Collaborative (NDCC) is hosting a webinar series on the six evidence-based comprehensive dementia care models that are part of NDCC.
In the fall, NCDD hosted a summit on "Strategies for Implementing Comprehensive Dementia Care: Resources for CMS GUIDE Participants and Other Dementia Care Providers." Summit participants had the opportunity to participate in an affinity group led by team members from one of six evidence-based comprehensive dementia care models that are part of the NDCC.
Each of the model teams are hosting additional affinity groups for anyone who is interested in learning more about their model. These meetings - which started in December 2024 and continue in early 2025 - are open to all and each will focus on model-specific implementation questions:
