The National Dementia Care Collaborative (NDCC) is hosting a webinar on practical strategies to strengthen care coordination, with insights from Johns Hopkins' Maximizing Independence (MIND) at Home program.

The webinar will showcase how MIND aligns assessments, home visits, care plans and respite care. It will also address the role of technology in connecting primary care providers and community organizations under the CMS GUIDE Model.
