Nominations Open for The John A. Hartford Foundation 2025 Business Innovation Award
USAging’s Aging and Disability Business Institute is accepting nominations for The John A. Hartford Foundation 2025 Business Innovation Award. Applications are accepted through May 28.
The award acknowledges the achievements of community-based organizations (CBOs) that partner and contract with health care entities, such as health systems, health plans, and accountable care organizations, with the goal of replicating these successful partnerships nationwide.
This year, the award will recognize community-based organization health care contracts and partnerships that have improved social connection—leading to better health outcomes and enhanced quality of life for older adults, people with disabilities and/or caregivers. Applications are especially encouraged that meet the following criteria:
- Promote social connection and/or address social isolation and/or loneliness,
- Are associated with a current health care contract and
- Include one or more of the Age-Friendly Health Systems 4Ms (what Matters, Medication, Mentation and Mobility).
The award is sponsored by JAHF and presented by USAging’s Aging and Disability Business Institute and engAGED: The National Resource Center for Engaging Older Adults.
Recipients will be announced and recognized at USAging’s 50th Annual Conference and Tradeshow on July 22 in Chicago, IL. The first-place winner will receive a monetary award of $5,000, and upto two runners-up will receive $1,000.
Submit your nominations by May 28.
