RIC Webinar: Strategies for Improving Care Coordination for Individuals with Intellectual and Developmental Disabilities
Resources for Integrated Care (RIC) is holding a webinar on August 9 on, "Strategies for Improving Care Coordination for Individuals with Intellectual and Developmental Disabilities (I/DD)."
The webinar will discuss approaches for providing coordinated care for individuals with I/DD, including person-centered medical assessment considerations for individuals with I/DD, risk stratification strategies to drive interventions, and family or caregiver support, education and outreach. Speakers will describe the current gaps in care for individuals with I/DD and share strategies for delivering person-centered care through use of interdisciplinary teams and innovative care coordination models. The event will include a live Q&A session.
