The National Center for Complex Health and Social Needs: Apply to Host a Regional Convening on Complex Care

The National Center for Complex Health and Social Needs has announced plans to provide six organizations with the opportunity to co-design and host a convening of regional complex care stakeholders across the country to address complex care barriers in their own communities.

In developing the Blueprint for Complex Care, The National Center heard the need for more local connectivity to create coordinated systems of care for individuals with complex health and social needs. This 2020 regional convening aims to address that concern by facilitating information sharing and collaboration on a regional level and fostering ecosystems of complex care.

The application is open until February 29, 2020.
