The National Dementia Care Collaborative 2025 Autumn Summit

2025 NDCC Autumn Summit Updated

The National Dementia Care Collaborative (NDCC) will hold Part 2 of its 2025 Autumn Summit on December 2 (rescheduled from October 28). This is a virtual event. Part 1 of the Summit was held in-person on November 11.

The Summit is designed to unite clinicians, community-based organizations, and national experts to discuss the early challenges of launching GUIDE, highlight proven strategies from the field, and respond directly to participants’ most pressing implementation questions.

The in-person gathering on Tuesday, November 11 in Boston (pre-GSA) featured a screening of the documentary Take Care, followed by thought-provoking conversations and interactive sessions focused on advancing real-world dementia care solutions.

The virtual forum on Tuesday, December 2, at 2 p.m. ET will bring together experts from the Center for Medicare and Medicaid Innovation to share what they've learned one year into the GUIDE program. Gain insights from people with lived experience and explore innovative approaches with dementia care model developers.
