USAging 2025 Success Stories: Award-Winning Health Care-CBO Partnerships
USAging's Aging and Disability Business Institute has published new success stories from the 2025 John A. Hartford Foundation Business Innovation Award winners, showing how community-based organizations and health care entities worked together to foster social connection, improve outcomes for older adults and provide models for communities nationwide.
- United Disabilities Services Foundation partnered with WellSpan Ephrata Community Hospital to support hospital-to-home transitions. The program reduced hospitalizations by 86%, while 57% of participants experienced improvements in depression and anxiety screening scores, which were linked to greater independence and community engagement.
- Senior Connection, Inc. partnered with UnitedHealthcare Community Plan of Massachusetts on a mobile health outreach program that reduced avoidable emergency department visits by 45% while connecting 92% of participants to a primary care provider.
- Elder Services of Worcester Area, Inc. partnered with Blue Cross Blue Shield of Massachusetts to help older adults transition home after emergency department visits, with 95% of participants saying post-discharge support was helpful.
