The Better Care Playbook is holding a webinar on December 12, "Coming Home and Staying There - Improving Care Transitions for Dually Eligible Beneficiaries."

Three plans will describe their innovative integrated health plan approaches to improve care transitions, including how they identify individuals in need of supports, work with delivery system and community-based partners, and address the needs of members with housing insecurity. Speakers will share program outcomes and lessons learned. The featured health plans participate in Promoting Integrated Care for Dual Eligibles (PRIDE), a project of the Center for Health Care Strategies made possible The Commonwealth Fund to help health plans provide high-quality care for dually eligible beneficiaries.

The Better Care Playbook, supported by a collaborative of foundations including JAHF, is an online resource of the best evidence-based practices and promising approaches for health care organizations participating in value-based payment models to improve outcomes and lower overall costs of care for people with complex needs.
