Ariadne Labs: Serious Illness Care Program COVID-19 Response Toolkit and Conversation Guides
As part of their Serious Illness Care Program COVID-19 Response Toolkit, Ariadne Labs has released a conversation guide for long-term care, along with a context sheet and a video demonstration of the guide.
Conversations between patients and clinicians about what matters most lead to higher quality care and improved quality of life for patients and those who care for them. The COVID-19 pandemic makes communication both more difficult and more important than ever, particularly for people who are at highest risk of becoming very sick. Ariadne Labs has developed the "Serious Illness Care Program COVID-19 Response Toolkit" to help patients have important conversations with people they trust within their support networks. Additionally, the toolkit includes resources for health systems and clinicians to address the communications needs of patients in the community and those in the hospital.
The long-term care guide, "COVID-19 Conversation Guide for Long-Term Care," assists health professionals in having conversations about values and care preferences with patients who reside in long-term care or other skilled nursing facilities or their caregivers.
