IHI Virtual Expedition: Conversation Ready - Engaging Patients in Advance Care Planning

The Institute for Healthcare Improvement (IHI) has launched a Virtual Expedition, "Conversation Ready: Engaging Patients in Advance Care Planning," which starts on July 16, 2019.

​​​​IHI Virtual Expeditions are action-focused online training programs for teams. The upcoming training focuses on becoming Conversation Ready, an effort to provide more patient-centered care by gaining an understanding of what matters most to patients when it comes to end-of-life care.

Structured around the aims of The Conversation Project — an initiative to ensure that every person's wishes for end-of-life care are expressed and respected — this virtual training teaches the five principles of being Conversation Ready.
