Informational Call: Age-Friendly Health Systems Action Community

The Institute for Healthcare Improvement (IHI) is holding an informational call on February 23 about the next Age-Friendly Health Systems (AFHS) Action Community that starts in March 2021.

The AFHS Action Community brings teams across different health systems together to accelerate their own description and adoption of the 4Ms: What Matters, Medications, Mentation, and Mobility. This informational call will provide a high-level overview of the AFHS movement and how to engage in the Action Community.

AFHS is an initiative of The John A. Hartford Foundation (JAHF) and IHI in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).
