JAGS: Three Papers on Patient Priorities Care
The Journal of the American Geriatrics Society (JAGS), a journal published by the American Geriatrics Society (AGS), has published two articles and an editorial in their October issue on Patient Priorities Care.
Patient Priorities Care is a model that aligns care among all clinicians with what matters most to their patients. It is particularly suited for older adults who must manage multiple conditions and navigate multiple clinicians.
The papers are:
- “Development of a Clinically-Feasible Process for Identifying Patient Health Priorities” (Naik et al.)
- “Feasibility of Implementing Patient Priorities Care for Patients with Multiple Chronic Conditions” (Blaum et al.)
- “Implementing ‘Person-Centered Care’ in Practice: A Revolutionary Change in Health Care Design and Delivery” (Applegate et al.)
The John A. Hartford Foundation co-funds implementation and testing of the Patient Priorities Care model.
