Dissemination of Geriatric Interdisciplanary Teams in Practice (GIT-P)

Grant Summary

One of the hallmarks of excellent geriatric care is coordination of multiple disciplines to meet the full range of needs of older adults. To that end, the Foundation approved this grant to stimulate and support the adoption through its five team-based models of care which were developed under the Geriatric Interdisciplinary Teams in Practice initiative. The project will develop top-quality commonly branded marketing and training materials for distribution, and project staff will provide technical assistance and consultation to interested health systems. The Foundation's previous work with the five projects in its Geriatric Interdisciplinary Teams in Practice initiative supported the creation and testing of innovative models of interdisciplinary team care. The five projects-Group Health Cooperative of Puget Sound, Intermountain Health Care, PeaceHealth Oregon Region, Rush University Medical Center, and the University of Colorado Health Sciences Center-will continue to work collaboratively on this dissemination effort.
