Moving and Scaling Home-Based Primary Care Phase II: Quality, Training and Advocacy
Grant Summary
This initiative aims to further improve the health of the most frail and vulnerable older adults living in the community by increasing access to high-quality, home-based primary care. Building on the success of the previous phase, three organizations will lead three coordinated grants:
- American Academy of Home Care Medicine, (PI: Eric DeJonge, $340,000), will develop a centralized, comprehensive directory of home-based primary care practices;
- Home Centered Care Institute, (PI: Thomas Cornwell, $385,000), will increase training of clinicians;
- National Home-Based Primary Care Learning Network, (PI: Christine Ritchie, Bruce Leff, $850,000 to Massachusetts General Hospital), will expand and enhance their National Learning Network for home-based primary care providers.
Together, these three coordinated projects will deliver measurably improved care and lead to improved outcomes for home-bound older adults and their caregivers.
