Age-Friendly Health Systems: Guide to Care of Older Adults in Nursing Homes and Other Nursing Home Resources
The Institute for Healthcare Improvement (IHI) has added a new Age-Friendly Health Systems (AFHS) Nursing Home Resources tab on the AFHS website. The resources include the updated Guide to Care of Older Adults in Nursing Homes, along with the companion Workbook for nursing home teams and Medical Director Recommendations to implement age-friendly care in nursing homes.
The updated and new materials include:
- Guide to Care of Older Adults in Nursing Homes: presents a systems-based approach to implementing the AFHS 4Ms Framework throughout the nursing home, includes - along with the workbook - specific references to nursing home regulations, assessments, and examples. Level 1 and Level 2 recognition explicitly built into the steps.
- Nursing Home Workbook: designed by and for point-of-care teams that interact with individual residents each day.
- FAQs document
- Building the 4Ms into Nursing Home Culture: Care Plan Conferences, Quality Assurance Performance Improvement (QAPI), and Stand-up Meetings: includes 3 short case studies of nursing homes that have integrated the 4Ms into a specific aspect of nursing home work/culture that are widely applicable across the U.S.
- Guide to the 4Ms in PointClickCare: includes screenshots and instructions for how to build 4Ms care plans and delirium screening into PCC—a common nursing home EHR platform.
- Medical Director Recommendations to Implement Age-Friendly Care in Nursing Homes: A Companion Document to IHI’s Guide to Care of Older Adults in Nursing Homes, designed to identify and describe opportunities for nursing home medical directors to co-lead and support implementation of age-friendly care with other leaders and members of the nursing home team.
