Join the AHA Age-Friendly Health Systems Fall 2026 Action Community
The American Hospital Association (AHA) is inviting health care teams to join the next Age-Friendly Health Systems Action Community, starting September 2026.
This free, seven-month virtual program brings together clinical teams across care settings to accelerate the adoption of evidence-based age-friendly care. Participants test the 4Ms Framework (What Matters, Medication, Mentation and Mobility) and share insights, with the goal of achieving recognition as an Age-Friendly Health System.
The Action Community includes monthly webinars, coaching calls, and access to practical tools and peer learning to help teams integrate age-friendly care into daily practice.
