Join the AHA Age-Friendly Health Systems Fall 2026 Action Community
The American Hospital Association (AHA) is inviting health care teams to join the next Age-Friendly Health Systems Action Community, convening September 2026 through April 2027.
This free, seven-month virtual program brings together clinical teams across care settings to accelerate the adoption of evidence-based age-friendly care. Participants test the 4Ms Framework (What Matters, Medication, Mentation and Mobility) and share insights, with the goal of achieving recognition as an Age-Friendly Health System.
The Action Community includes monthly webinars and access to expert coaching, valuable resources and peer learning to help teams integrate age-friendly care into daily practice.
Join an August 6 AHA webinar, "Putting the 4Ms into Practice," to hear the director of nursing at Morristown Medical Center share the hospital’s age-friendly care journey.
