Better Care Playbook for People with Complex Needs: Refreshed Website
The Better Care Playbook has launched its refreshed website that makes it easy for stakeholders to find what works to improve care for people with complex health and social needs.
Since 2017, the Playbook has served as an online resource for complex care stakeholders looking to find evidence-based and promising practices for improving care. It is made possible by seven leading health care foundations — Arnold Ventures, The Commonwealth Fund, The John A. Hartford Foundation, the Milbank Memorial Fund, Peterson Center on Healthcare, the Robert Wood Johnson Foundation, and The SCAN Foundation — that are working together to accelerate health system transformation.
New features on the Playbook website include:
- What is Complex Care? – More about adults with complex needs, how complex care can help address their needs, and the business case for complex care.
- Resources by Topic – A range of issue areas important to stakeholders in the field — such as Complex Care Interventions, COVID-19, Interdisciplinary Care Teams, and Payment & Managed Care Models.
- Perspectives on Better Care Blog – Compelling viewpoints from on-the-ground complex care providers and thought-leaders in the field.
