Call for Applications: Hartford Change AGEnts Action Awards
Hartford Change AGEnts Action Awards are one-year grants for up to $10,000 available to interprofessional teams led by Hartford alumni for the purpose of achieving meaningful change to practice or policy that will improve the health and wellbeing of older adults and/or their families.
Hartford Change AGEnts Action Awards are one-year grants for up to $10,000 available to interprofessional teams led by Hartford alumni for the purpose of achieving meaningful change to practice or policy that will improve the health and wellbeing of older adults and/or their families.
For this initiative, we view changing practice as a collaborative and interprofessional process that uses strong evidence to achieve results. Proposals should reflect the involvement of the proposed “adopter” organizations and other stakeholders in the project.
The Change AGEnts Initiative expects to fund five to eight grants under this announcement.
