There is an upcoming Care Management Plus Training partially supported by the California HealthCare Foundation. The conference will be May 3rd and 4th, at the Hilton Garden Inn San Francisco/Oakland Bay Bridge Hotel in Emeryville, California. The cost is $1500 per person, includes 2 days in-person and 8 weeks online learning, and emphasizes Motivational Interviewing. ANCC credits are provided. There are openings for care managers, social workers, health educators, physicians, managers, and healthcare leaders who are interested in attending. If you are working with clinics or groups who are becoming medical homes, please let them know about the training. The team would welcome their participation and appreciate any ideas you have to help us reach them.
Care Management Plus, funded in part by the John A. Hartford Foundation, leverages information technology in primary care clinics to help care managers and an interdisciplinary team improve care coordination, identify and address the health issues of individual patients and patient populations, and leads to better health and lower health care spending.
