The Collaborative Action Team training for Community Health - Older adult Network (CATCH-ON GWEP) has released new learning videos about the 4Ms of the Age-Friendly Health Systems initiative.

The brief videos help older adults and families learn about the 4Ms of Age-Friendly Health Communities (What Matters, Medication, Mind, and Mobility) with videos on caregiving, delirium, dementia, depression, medication and mobility. There are also links to additional resources on the 4Ms.

CATCH-ON GWEP has also released professional modules that include a version for all health system employees including non-clinicians and clinicians - all with free CE - on the Basics of Aging including an introduction, evaluating memory concerns, normal aging vs. disease, managing multiple chronic conditions, working with teams of health care clinicians and person-centered dementia care in a hospital setting.

CATCH-ON is the name of Rush University Medical Center’s College of Nursing and Center for Excellence in Aging's Health Resources and
Services Administration (HRSA) Geriatrics Workforce Enhancement Program (GWEP).
