CHCS Evidence to Action Collection Supporting Older Adults in Rural Communities

The Center for Healthcare Strategies (CHCS) has published a new Evidence-to-Action Collection on "Supporting Older Adults in Rural Communities: Home- and Community-Based Care."

Rural communities — home to more than 20 percent of older adults in the U.S. — face significant barriers to health care access, including limited transportation, care facility closures, and ongoing workforce shortages. Despite these challenges, most older adults prefer to age in place, and achieving this goal requires targeted strategies that build on the strengths of rural areas to support aging at home.

This collection features three proven care models that can each be tailored to the strengths and culture of rural communities to better serve older adults:

  • Program of All-Inclusive Care for the Elderly (PACE)
  • Community-based palliative care, and
  • Community paramedicine

State policymakers, plans, providers, and community organizations can use this collection to review emerging evidence, case examples, practical tools, and policy considerations to implement these models in rural settings.
