sponsored by the Centers for Medicare and Medicaid Services (CMS) and the Center for Medicare and Medicaid Innovation (CMMI). The purpose of the daylong event was to showcase and spread leading innovations that support the triple aim of better health, better care and lower cost. Berman spoke of the role of patients in achieving the triple aim. Panelists also included Hartford grantee, Mary Naylor, discussing theTransitional Care Model.
