HCCI Publication, Health Care’s Perfect Storm and the Home-Based Primary Care Solution
The Home Centered Care Institute (HCCI) has released a new publication called Home-Based Primary Care’s Perfect Storm: Health Care’s Perfect Storm and the Home-Based Primary Care Solution.
HCCI is a collaborative nonprofit led by Dr. Cornwell that is advancing home-based primary care nationwide through training, innovation, and research. Home-based primary care is a model of care that brings the expertise of nurse practitioners, physicians and physician assistants directly to the patient in the comfort of their own home.
The white paper addresses how home-based primary care benefits both providers and patients and why the industry is perfectly poised to advance this effective care model.
