Hospital at Home Users Group Technical Assistance Center
The Hospital at Home Users Group, in partnership with the Center to Advance Palliative Care (CAPC), has gathered an array of practical help into a one-stop Hospital at Home Users Group Technical Assistance Center.
Users can access resources and information via the Technical Assistance Center to help start, build, or expand a high quality Hospital at Home (HaH) program. Information includes case studies and implementation tools, links to resources on special topics, as well as HaH's webinar series, and information on the CMS Acute Hospital Care at Home waiver and program. Office Hours are now available in the Technical Assistance Center, where you can meet in small groups with Hospital at Home Users Group leaders and get useful insights into the challenges starting or growing a hospital at home program.
The resources are grouped under five areas:
- CMS Waiver
- Indicates whether your organization can apply, and how to approach the application
- Stakeholder Engagement and Program Planning Basics
- Provides users with tools to help design stakeholder engagement strategies
- Patient Eligibility, Referral, and Intake
- Offers guidance on designing patient eligibility criteria, including sample inclusion and exclusion criteria, screening guides, and patient consent forms
- Program Operations
- Offers resources on staffing, clinical operations, pharmacy, measurement and quality, technology and EHR, and supporting caregivers
- Financial Models, Payer Partnerships, Program Growth and Sustainability
The site is frequently updated with case studies, implementation tools, links to resources, and information on the CMS Acute Hospital Care at Home waiver and program.
