HRET's Huddle for Care: Series of Articles on Age-Friendly Health Systems
The American Hospital Association's non-profit affiliate, HRET has published a series of articles on Huddle for Care about Age-Friendly Health Systems, including, "Rolling Out the 4Ms for Age-Friendly Care Across the Care Continuum."
The articles are written by nurses, social workers and hospital administrators and provide detailed insights into ways that specific health systems are making all their work with older adults age-friendly. Impact of this work is discussed from a staff, patient and health system perspective, and each article includes tips on lessons learned.
