IHI/ASCP Webinar: Empowering Pharmacists to Deliver Age-Friendly Care
The Institute for Healthcare Improvement (IHI) and the American Society of Consultant Pharmacists (ASCP) are hosting a webinar exploring how pharmacists and pharmacies can help advance age-friendly care through the 4Ms Framework (What Matters, Medication, Mentation, Mobility).
ASCP will highlight new opportunities for pharmacists and pharmacies to engage with the Age-Friendly Health Systems movement through the Age-Friendly Pharmacist Badge Program and Age-Friendly Pharmacy Recognition. In addition, speakers from recognized community and long-term care pharmacy settings will share practical strategies and lessons learned from implementing age-friendly practices.
