Caring for the Caregivers is Part of Optimal Age Friendly Care IHI Blog 1

The Institute for Healthcare Improvement (IHI) has released a blog, "Caring for the Caregivers is Part of Optimal Age-Friendly Care," about their Caregiver Intervention (4Ms-CGI) program.

The blog talks about IHI's commitment to supporting caregivers of older adults through their recent partnership with Rush Medical Center on a pilot program, the Caregiver Intervention (4Ms-CGI) program. The blog focuses on one of the six pilot sites, Jamaica Hospital Nursing Home (JHNH). JHNH is part of Jamaica Hospital Medical Center (JHMC), one of the leading health systems in Queens, New York, and a participant in the Age-Friendly Health Systems initiative.

Rush Medical Center's 4Ms-CGI program has two goals:

  1. Transform how staff think about and interact with family caregivers
  2. Provide programs and services for family caregivers to address their own needs and help them more effectively support the older adults in their lives.

The blog explains the process of the program, from meeting and getting to know the caregiver, how they assessed their needs and the tools used, and the discharge process where residents are provided a checklist of things to teach the caregiver before they go home. The final part of the program is following up with the caregivers after the end of the intervention. Next steps going forward for the team are also discussed.
