On April 25th, the Bipartisan Policy Center (BPC) will be releasing the second in a series of reports designed to improve care for high-need, high-cost Medicare beneficiaries.

Presentation of the Improving Care for the Chronically-Ill Medicare Patients report findings will start at 9am and will be followed by a panel discussion. Speakers will include:

Building on a recent report on improving care for individuals dually eligible for Medicare and Medicaid, BPC's second report will outline the barriers to the integration of clinical health and social services and issue recommendations to eliminate those barriers for the Medicare-only population.
