Innovation Center Announces Round II Community Based Care Transitions Sites
The CMS Center for Medicare Innovation announced the next 23 Community-based Care Transition Program (CCTP) demonstration sites, bringing the total to 30. The demonstrations are funded under section 3026 of the Affordable Care Act. Twenty-four sites are basing their work on the Care Transitions Intervention model developed with Hartford Foundation funding by Eric Coleman. Read more here.
