The National Alzheimer’s and Dementia Resource Center (NADRC) is hosting a webinar on February 23, "From Dementia to COVID-19: Maintaining Innovations using Data Driven Program Decision Making," about an innovative dementia care transitions program.

During the COVID-19 pandemic, the Nevada Senior Services Hospital2Home (H2H) program successfully provided discharge relief and prevented unncessary readmissions for complex and vulnerable populations. This webinar will provide an overview of the H2H program, how it served people living with dementia and their caregivers during the pandemic, and how its data-driven approach determined its success, expansion, and sustainability.
