2025 National Healthcare Decisions Day 3

National Healthcare Decisions Day (NHDD) is observed annually on April 16 to raise awareness, educate, and encourage individuals and healthcare professionals about the value of advance care planning.

Led by The Conversation Project (TCP), NHDD aims to motivate people to communicate their healthcare preferences and ensure that medical providers and institutions honor those decisions. TCP offers a variety of Advance Care Planning tools and resources on its website, including:

  • PREPARE For Your Care is an interactive online platform designed to support individuals as they make medical decisions. PREPARE For THEIR Care offers guidance and resources specifically for caregivers.

  • The Serious Illness Messaging Toolkit is a digital guide for effectively communicating with the public about care for those with serious illnesses.
  • FAIR Health provides practical tools and cost-related healthcare information tailored to older adults and their caregivers.
