Hartford Foundation, about what “age-friendly care” is and where you can find it. They explain about asking for what matters to you when you visit your doctor or hospital. What matters is one of the 4Ms of the Age-Friendly Health Systems initiative: What Matters, Medication, Mentation, and Mobility.
