Resources: Age-Friendly Care and Education Collection from the Minnesota Northstar GWEP
The Minnesota Northstar Geriatrics Workforce Enhancement Program (GWEP) has developed "The Age-Friendly Care and Education Collection," an online set of clinical and educational resources for health professionals, educators, learners, and others interested in improving care for older adults.
This resource collection includes geriatric education toolkits/modules, case studies, clinical tools, preceptor materials, patient education materials, journal articles, videos, and more.
The Age-Friendly Care and Education Collection is created in collaboration with the National Center for Interprofessional Practice and Education. It is part of Minnesota Northstar GWEP at the University of Minnesota, supported by the Health Resources and Services Administration (HRSA) Geriatrics Workforce Enhancement Program, the University of Minnesota Office of Academic Clinical Affairs, and the Otto Bremer Trust.
