The Better Care Playbook: Telehealth in Complex Care Collection
The Better Care Playbook (BCP) has published a collection, "Telehealth in Complex Care," which explores what works in complex care telehealth interventions, what successful models look like in practice, and implementation strategies.
Through the curation of peer-reviewed research, case studies, tools, and on-the-ground perspectives, this BCP Telehealth in Complex Care collection helps to address what works, what it looks like in practice, and how an organization can implement this care model. The accelerated adoption of telehealth amid the pandemic demonstrates that virtual care could keep people safe from infection and help keep them on track with their health care. Many providers and patients enjoy the flexibilities offered by telehealth, but some individuals with complex health and social needs may still experience barriers to accessing it.
Healthcare stakeholders can use this resource to understand recent evidence behind telehealth-delivered interventions and learn about practical strategies to support effective implementation of these programs for people with complex health and social needs.
