Webinar: The Hospital at Home Model and the CMS Acute Hospital Care at Home Waiver

The American Academy of Home Care Medicine (AAHCM) and the Hospital at Home Users Group are holding a webinar December 17 on, "The Hospital at Home Model and the CMS Acute Hospital Care at Home Waiver."

The webinar will focus on rationale and evidence base for Hospital at Home, key features of the Hospital at Home care model, and the recent Center for Medicare and Medicaid Services (CMS) Acute Hospital Care at Home initiative. Health care professionals interested in learning about the Hospital at Home care delivery model and the recent CMS initiative in this area are encouraged to attend.

Presenters include:

  • Bruce Leff, MD, Professor of Medicine, Johns Hopkins University School of Medicine. Dr. Leff is a geriatrician and health services researcher and has led Hospital at Home development, evaluation, and dissemination efforts in the U.S. since the 1990s.
  • Linda DeCherrie, MD, Professor of Medicine, Icahn School of Medicine at Mount Sinai. Dr. DeCherrie is the Clinical Director, Mount Sinai Hospitalization at Home.
