Invitation to Join: Age-Friendly Health Systems Action Community

The Institute for Healthcare Improvement (IHI) is inviting health systems to join the next Age-Friendly Health Systems Action Community that starts in March 2024.

Age-Friendly Health Systems (AFHS) follows an essential set of evidenced-based practices called the 4Ms: What Matters, Medication, Mentation and Mobility. The seven-month AFHS Action Communities provide a series of monthly webinars and coaching calls to support teams to learn about, test and adopt the 4Ms in their care setting.

This Action Community begins in March and registration is open through March 13. IHI is especially encouraging hospitals (particularly critical access), nursing homes and Federally Qualified Health Centers to join.
