Diagnostic Excellence Age-Friendly Health Systems (AFHS) Seed Grants Program

Grant Summary

This grant will support the development and prototyping of innovative solutions that reduce the burden of diagnostic error and delay on older adults and their families through a seed grants program for health care delivery systems and researchers. Over two overlapping cohorts, up to 16 health care organizations will receive funding for quality improvement and research projects that improve the quality, accuracy, and timeliness of diagnoses for older adults. A second track will invite approximately 12 health care organizations with the most promising interventions to continue in a demonstration collaborative. The initiative will recruit candidates from both the seed grant program and the Age-Friendly Health Systems movement, also led by the Institute for Healthcare Improvement.

This grant was originally awarded to the Society to Improve Diagnosis in Medicine (SIDM).
