Age-Friendly Health Systems Resources on Identifying and Documenting Caregivers of Older Adults
The Institute for Healthcare Improvement (IHI) has released two new Age-Friendly Health System (AFHS) resources on identifying and documenting caregivers of older adults.
These tools were designed to help health systems begin the caregiver engagement journey:
- A guide, "Tips to Identify and Document Caregivers of an Older Adult," that covers ways health systems can get started, how to ask about caregivers, how to document caregivers in the electronic health record, and ways to continue the work and engage.
- An accompanying slide deck, "Identify and Document Caregivers of Older Adults: How to Get Started," which includes resources, case examples, testimonials and sample tools from health care sites.
