The University Center on Aging and Health and the Frances Payne Bolton School of Nursing, Case Western Reserve University are hosting a webinar April 29, "Age-Friendly Health Systems - Driving Change and Outcomes." The webinar is part 1 of the 2021 Florence Cellar Webinar Series.

The webinar will cover the following learning outcomes:

  • Describe the Age-Friendly Health Systems initiative funded by The John A. Hartford Foundation and supported by the Institute for Healthcare Improvement.
  • Identify two resources for the implementation of Age-Friendly Health Systems into your organization.
  • State how the Age-Friendly Health Systems 4Ms Framework is integrated into MinuteClinic, the retail medical clinic of CVS Health.
